SmartHealthConnect turns Claude into a patient-facing health copilot. Six auto-discoverable skills invoke themselves based on natural prompts: • care-completion — tracks preventive screenings, referrals, and follow-ups against HEDIS and USPSTF quality measures. Triggers on "what screenings am I due for", "show my care gaps". • medication-refills — monitors refill windows, requests refills, and projects 90-day timelines. Triggers on "refill my lisinopril", "which meds am I about to run out of". • healthy-habits — a longitudinal health operating picture integrating sleep, exercise, medication adherence, vitals, and goal progress. • diet-exercise — logs activity and meals and analyzes correlations between exercise/diet and clinical outcomes (BP, HbA1c, weight). • kids-health — CDC immunization schedules, well-child visit cadence, and state-specific school health compliance. • research-monitor — filters bioRxiv / medRxiv preprints, ClinicalTrials.gov, and OpenFDA drug-safety signals to the patient's actual conditions and medications. Built on SMART on FHIR with PHI redaction, audit logging, and human-in-the-loop on write tools (add_journal_entry, generate_care_plan, generate_appointment_prep). Pairs cleanly with healthclaw-guardrails for the backend FHIR layer.
